Herniated discs in the neck and low back are fairly common. Sometimes a herniated disc will heal on it’s own. Sometimes conventional therapies such as chiropractic or physical therapy will help resolve a herniated disc. Other times nothing seems to help.

The symptoms can vary case to case. The most common symptoms are neck or low back pain, arm or leg pain, numbness, tingling in the extremities, weakness, night pain, carpal tunnel symptoms and even headaches. Everyone presents differently.

Spinal decompression therapy is performed on a machine that is designed and built to treat herniated and bulging discs naturally. The machine we use is the DRX9000.

The DRX9000 works by elongating the spine, enlarging the disc space, re-hydrating the disc, re-positioning the disc into a more normal alignment, strengthening the spinal ligaments, and restoring range of motion to the spine.

Sciatica Pain Treatment San Francisco Sciatica describes leg pain, numbness, or weakness in the legs, that is caused by pressure on the sciatic nerve in the low back or buttocks. The sciatic nerve is the largest nerve in the body. In some people the sciatic nerve is the diameter of a quarter at it's largest point.

Sciatica is usually caused by a pinched nerve in the lumbar spine from a herniated disc or a bulging disc. Degenerative disc disease and Facet syndrome can also cause sciatica…as can Piriformis syndrome. There is a muscle in the buttocks called the Piriformis muscle that the sciatic nerve passes through on it's journey down the leg to the feet.

Some people that sit a lot like truck drivers or office workers are prone to Piriformis muscle irritation, especially if they have a wallet in their back pocket which can cause postural imbalances and piriformis muscle spasms.

Spinal stenosis, which is a narrowing of the spinal canal or nerve root channels (secondary to disc degeneration or bulging or herniated disc pathology) can also cause sciatica symptoms.

Sciatica is usually an indication that your back condition is advancing and getting worse.

So what's the best treatment for sciatica pain?

Well, it depends who you talk to. Many orthopedic surgeons will want to either to cortisone injections to get the inflammation down…then physical therapy. Or, if the disc herniation in the low back is severe and the spine is unstable and has a lot of degeneration, they will want to perform back surgery.

A chiropractor will probably want to try a short course of spinal adjustments, massage, and exercise rehabilitation.

A spinal decompression doctor may want to do a combination of any of the above, or simply try spinal decompression as a stand alone treatment for your herniated disc, stenosis, bulging disc, or facet syndrome.

Lumbar Disc Decompression Without Surgery

Up until recently, lumbar disc decompression referred to a surgical procedure whereby part of the spine was cut-out to remove pressure off the spinal cord or lumbar nerve roots.

Now, the lumbar disc decompression or spinal disc decompression terms refer to procedures that can be either surgical or nonsurgical.

At Executive Express Chiropractic in the SF Financial District, we perform nonsurgical spinal decompression with a machine called the DRX9000. The DRX9000 was designed and built to treat neurovascular compression syndromes such as herniated discs, bulging discs, facet syndrome, and disc degeneration….without surgery (watch this spinal decompression success story video).

Nonsurgical disc decompression helps many patients avoid back surgery, but you have to qualify medically (How does spinal decompression work?).

If you do qualify for natural disc decompression then there is a very good chance it will help you.

Will you get completely better? Sometimes…but in most cases (at Executive Express Chiropractic) you simply become much better than you were. It just depends what we are starting with, your age, your over-all physical condition, how much spinal degeneration there is, how many disc herniations you have,  and your level of commitment to get better. 

Nonsurgical spinal decompression with the DRX9000 or one of the other excellent spinal decompression machines on the market might just be the answer you are looking for. And let me give you a piece of advice…Don't listen to the naysayers.

Nonsurgical spinal decompression is helping thousands of patients every day return to more normal lives and avoid surgery. And the only way to find out if it will work for YOU is to try it (but you must qualify).

Here's the deal…Spinal decompression is very safe…and it's gentle too. Sure there have been some rare cases where patients get really sore or think the machine made them worse…but it's extremely rare. In fact, we can count on one hand the amount of times this has happened and we have performed thousands of decompression sessions for herniated lumbar discs (and cervical discs).

Either spinal disc decompression works (which is most likely) or it doesn't. We don't cut anything out or inject anything in (to your body). If decompression therapy does not work you are still left whole, and you can pursue other options.

If you have certain kinds of back surgery such as a surgical fusion, you are disqualified from ever trying nonsurgical spinal decompression. It's something to think about.

My Take: If you have a condition such as a herniated lumbar disc, bulging disc, spinal stenosis, degenerated disc, facet syndrome, sciatica, back pain, leg pain…or any other lumbar or cervical spine disorder that is not responding to conventional back pain therapies (including chiropractic)…natural disc decompression is worth a look. You can always have surgery. But if you have surgery….you may not be able to try nonsurgical spinal decompression…ever.

Spinal Decompression for Sciatica

We have been helping patients with sciatica since 1992 at Executive Express Chiropractic (formerly Front Street Chiropractic) in the Financial District. 

About 5 years ago we began to incorporate spinal decompression into our low back pain protocols for sciatica. Most cases of sciatica are due to neurovascular compression from a bulging disc or herniated disc.

The term sciatica describes the symptoms of leg pain and possibly tingling, numbness or weakness that originates in the low back and travels through the buttock and down the large sciatic nerve in the back of the leg.

The vast majority of people who experience sciatica get better with time (usually a few weeks or months) and find pain relief with non-surgical sciatica treatment. For others, however, sciatic nerve pain can be severe and debilitating.

The Sciatic nerve is the largest nerve in the body…it's diameter can be as large as a quarter. The nerve roots of the sciatic nerve originate in the lower back and exit from in between the lumbar vertebrae, before coming together to form the sciatic nerve.

Nerves are very sensitive to pressure. In fact, the the weight of a dime on the sciatic nerve can interfere with the transmission of nerve impulses and cause pain. There are many sources of nerve root pressure, here are a few: Bone out of place (vertebral subluxation), spinal stenosis, disc herniation, disc bulge, spinal degeneration, facet syndrome, and facet syndromes.

When we get pressure on a nerve root, the pain will travel to whatever body part that nerve is traveling to. Since lumbar nerve roots travel down the leg and into the feet…pressure on them (often referred to as a pinched nerve) can cause pain, numbness, tingling, burning, weakness, in the legs and feet.

In reality, Sciatica is a symptom, not a cause. You can also get sciatica from pressure on the sciatic nerve as it travels through the Piriformis muscle in the buttocks…this is called Piriformis Syndrome.  

Since most case of sciatica are from a herniated disc or a bulging disc (often referred to as slipped disc)…the question becomes…What is the best treatment to fix a herniated disc?

Sure there is back surgery…but back surgery is not indicated all of the time. Heck…my experience has been that back surgery is not indicated most of the time.

Nonsurgical spinal decompression machines are designed and built to treat lumbar disc herniations, disc bulges, stenosis, and facet syndromes without surgery. Spinal decompression enlarges the disc space, elongates the spine, re-positions the disc, re-hydrates the disc, and strengthens the spinal ligaments and muscles. It's like a respirator for the discs. Sciatica from a Lumbar Disc Herniation?

We have been helping patients with sciatica in the SF Financial District for many years now at Executive Express Chiropractic.

Pain resulting from irritation to the sciatic nerve (the largest nerve in the body) is called sciatica. The primary  symptom of sciatica is pain in the leg. The pain can travel from the low back into the buttocks. The pain can travel all the way down the back of the leg (or side of the leg) or just part of the way…say to the knee. You can even have pain that travels down both legs. Sometimes there is numbness, tingling, and weakness along with the leg pain. And, there is usually associated low back pain…but it's not uncommon to have sciatica with no back pain.

The most common cause of sciatica is pressure on the lumbar nerve roots where the nerves exit (the low back) that form the sciatic nerve. And the most common cause of lumbar nerve root pressure is bulging or herniated discs. Disc degeneration, spinal arthritis, and spinal stenosis can also cause sciatica…as can piriformis syndrome or facet syndrome.

No matter what the cause…sciatica is literally a pain in the butt. It is not much fun at all. And sciatica can become chronic and disabling. And it can be a nightmare trying to find the right kind of treatment. Often times nothing seems to help. Some patients have tried everything from acupuncture, physical therapy, cortisone, pain meds…even chiropractic…nothing helps and they don't want back surgery. What can be done?

Well…there is a treatment that has been around for about 7-10 years or so…but has recently been refined to the point where it is highly effective for the nonsurgical treatment of lumbar disc herniations…it's call nonsurgical spinal decompression. The machine we use at our San Francisco Spinal Decompression Center is the DRX9000.

We chose this machine (after researching all of them) because it seemed to offer the greatest amount of patient comfort (they all offer similar outcomes). The DRX9000 is the Rolls Royce of spinal decompression machines…at least in my opinion.

Anyway, the DRX9000 is able to repair (without drugs and surgery) many bulging discs or herniated discs in the lumbar spine. It does this by re-positioning the injured disc, re-hydrating the disc, enlarging the disc space, elongating the spine, and pulling the herniated disc material off the lumbar nerve roots…which can be a blessing for your sciatica (see DRX9000 Special Report for treatment outcome studies).

However, the DRX is not a magic bullet.  It takes 20-26 one hour sessions, extensive home care and exercises…and does not work on every single person that does it. And…you have to qualify medically for spinal decompression…not everyone does. 

But here's the deal…The DRX9000 spinal decompression system is very safe and gentle. So even if the DRX fails to deliver results…which is the exception not the rule…you are still whole. Nothing has been removed from your body. Nothing has been fused. Nothing has been injected into you…it just didn't work and you can explore other treatment options. If you can handle this than the DRX9000 might just make sense for you.

San Francisco Low Back Pain Treatment

Images[9] I think low back pain is the #2 reason patients go to the doctor for musculoskeletal pain…headaches is #1. Low back pain is certainly the most common condition we see at Executive Express Chiropractic in downtown SF…then neck pain, headaches, carpal tunnel, and upper back and shoulder pain.

There are many ways to treat low back pain. There are pain medications, muscle relaxers, and anti-inflammatory pills and shots (cortisone). There is physical therapy, acupuncture, massage, rolfing, exercise, psychotherapy, and chiropractic. And there are thousands of gimmick therapies and pieces of equipment…as well as magic potions, lotions, and goose grease.

So how do you choose? Well…fortunately…chiropractic (in my opinion) seems to get the best results when it comes to chronic or acute low back pain. At least that's what our San Francisco low back pain patients tell us. Many have tried everything under the sun. Some just did a little research online or talked to some friends and came straight to us.

But here's the deal…even the unique brand of modern chiropractic that we do at Executive Express wont help everyone. I would say maybe 10-15% of the SF low back pain patients we treat need more (or something different) than chiropractic, ART, Graston, massage and exercise.

For these patients we have a next step…nonsurgical spinal decompression. Decompression therapy is a very advanced method of reducing disc bulges and disc herniations without drugs or surgery. The spinal decompression machine we have treats cervical and lumbar neurovascular compression syndromes and costs $150,000.

Non-Surgical Spinal Decompression or Physical Therapy for a Herniated Disc?

So how do DRX9000 disc herniation treatment outcomes stack up against the outcomes of physical therapy? 

Honestly…I don't know. What I do know is this…it sure seems like just about every patient we treat on the DRX9000 lumbar decompression system or the DRX9000c cervical decompression system, has already tried everything under the sun…including physical therapy. Hey…we do physical therapy also…but we do it when the spine is ready for it…after the herniated disc has been repaired, re-hydrated and repositioned.

Exercise just doesn't seem to work well (on some patients) when a herniated disc is putting pressure on a nerve or the spinal cord and there is lots of inflammation and spasm.

At least it didn't (physical therapy) on our disc patients that tried it (prior to DRX).

Now,  if you have just been diagnosed with a herniated disc and your medical doctor has prescribed physical therapy, should you do it?  Probably…otherwise you will never know if it would have helped…because it might. Same with chiropractic.

Hey…as I have said many times…not all herniated disc patients need nonsurgical spinal decompression…only those that do not respond to more conventional therapies such as physical therapy, chiropractic, acupuncture, and medical treatments.

Heck…some of you will start to feel better if you do nothing.

Sure, some patients jump right on the DRX without trying anything else…and it's probably a good choice (yes my opinion is biased)…but most run the course and try all the aforementioned treatments…probably because decompression is NOT covered by insurance and all the other treatments are. The DRX9000 has just not been around long enough to be covered yet.

Now that there is favorable preliminary research…it will lead to long term studies…that will eventually lead to nonsurgical spinal decompression being covered by insurance. But who wants to wait?

Anyway…the bottom line is this…If you feel like you have tried everything for your herniated disc…or want to save the time of trying everything and try the most effective therapy first…nonsurgical decompression with the DRX9000 just might be the answer.
