How Long Do Chiropractic Adjustments Last?

We have been providing chiropractic care in San Francisco for over 24 years.

Some of the more common questions patients have when they visit a chiropractor for the first time are as follows:

1. How many adjustments do I need?

2. Are adjustments safe?

3. How long will the adjustments last and will I have to keep coming in forever?

Well, these are great questions. After-all, we all like to know what we are getting ourselves into, and nobody likes expensive surprises.

The good news is that chiropractic is not expensive and is covered by most health insurance plans. Chiropractic is also very safe and effective…which is why insurance plans cover it. What dictates how many adjustments a patient needs is the nature of the presenting complaints, whether or not the condition is new and acute or chronic (over 3 months), the patients overall health status, and any complications such as scoliosis or diabetes.

Insurance companies have guidelines based on the above and most doctors adhere to these even for cash patients. The chiropractic technique of your doctor is part of the equation also. Some techniques do more corrective type care which takes longer but is longer lasting. Some do relief care which is just treating the symptom and getting the patient out of pain. This is what most patients want.

How long your chiropractic adjustments last depends on all the above and your adherence to the treatment plan and home care recommendations like stretches and exercises. It also depends on your lifestyle, including your work load, your stress levels, your diet, how much you exercise, hobbies, etc.

Some patients go years without adjustments. Some want them a few times a month. Chiropractic is a tool you can use as needed to help you express your health potential. Once you get the hang of it you figure out what works best for you. You make the choice.

Back, Neck and Arm Pain Treatment in San Francisco

Chiropractor San Francisco Financial DistrictOur "Patient of the Week" is Jennifer Rocco. We first met Jennifer almost 20 years ago. She had developed back, neck, and arm pain from long hours of repetitive movements at work. Jennifer works for a gaming company and is involved in testing new products and research and development. The Neck and Headache Connection

Headache clinic san franciscoWhen we hear the term headache, we don’t usually think about the neck. Rather, we focus on the head, more specifically, “…what part of the head hurts?” But, upon careful questioning of patients, our San Francisco Headache Doctors usually find some connection or correlation between neck pain and headaches.

The key to this connection can be found in looking at the anatomy of the neck. There are 7 vertebrae that make up the cervical spine and 8 sets of nerves that exit this part of the spine and innervate (connect to and control) various parts of the head, neck, shoulders and arms, all the way to the fingers. Think of the nerves as electric wires that stretch between a switch and a light bulb. When you flip on the switch, the light illuminates. Each nerve, as it exits the spine, is like a switch and the target it travels to represents the light bulb. So, if one were to stimulate each of the nerves as they exit the spine, we could “map” exactly where each nerve travels (of course, this has been done).  Cervical nerves and headaches
When we look specifically at the upper 3 sets of nerves that exit the spine (C1, C2, and C3), we see that as soon as they exit the spine, they immediately travel upwards into the head (the scalp). Like any nerve, if enough pressure is applied to the nerve, some alteration in nerve function occurs and usually a sensory change is noted (numbness, tingling, pain, burning, etc.). If the pressure continues, these symptoms can last for a long time. These types of headaches are often called “cervicogenic headaches” (literally meaning headaches that are caused by the neck). These can be caused by the nerves getting pinched by tight muscles through which they travel as they make their way to the scalp.

Forward head postureThese muscles are usually tight in people that spend the majority of their day in front of a computer…especially if they have developed Forward Head Posture.

Another connection between the neck and headaches includes the relationship between 2 of the 12 cranial nerves and the first three nerves in the neck described above. These types of headaches usually only affect one half of the head – the left or right side. One of the cranial nerves is called the trigeminal nerve (cranial nerve V). Because the trigeminal nerve innervates parts of the face and head, pain can also involve the face. Another cranial nerve (spinal accessory, cranial nerve IX) can also interact with the upper 3 cervical nerve roots, resulting in cervicogenic headaches. People with cervicogenic headaches will often present with an altered neck posture, restricted neck movement, and pain when pressure is applied to the base of the skull or to the upper vertebrae. Other than a possible numbness, there are no clinical tests that we can run to “show” this condition, though some patients may report scalp numbness or, it may be found during examination.

Though medication, injections, and even surgical options exist, manipulation applied to the small joints of the neck, especially in the upper part where C1-3 exit, works really well so why not try that first as it’s the least invasive and, VERY EFFECTIVE! In some cases, a combination of approaches may be needed but many times, chiropractic treatment is all the patient needs for a successful outcome.

Is Chiropractic Safe?

Is chiropractic safe I just received a question from a blog reader asking "If Chiropractic Was Safe?"…so I decided to write a post about it. I have read that the chances of getting struck by lightning are greater than suffering a stroke from a chiropractic adjustment (a rare event)…which is what most are concerned about.

Heck…more people have strokes while driving or getting their hair shampooed at a hair salon than in a chiropractic office. Having a stroke is more about risk factors and lifestyle choices than chiropractic adjustments.

My malpractice insurance which is for the highest of limits is only $2000 per year. And we all know that insurance rates are based on incidence statistics.

Sure…it's possible to get sore from chiropractic adjustments…after-all it is physical. Our San Francisco Chiropractors try to reduce the odds of soreness by always starting out light and building up the intensity slowly per patient tolerance. We use ice at at the clinic and at home to reduce the likelihood of soreness.

We are also careful to consider the age, over-all physical condition, and pain levels of the individual patient. Patient comfort is a primary concern of ours. In fact…our chiropractic patients tell us they really look forward to their adjustments…especially the warm-up stretches and massage.

Yes, there is always the chance someone can get really sore…but it is the exception and not the rule.

Heck…if you start a new exercise program you can get sore. But if you are smart, you start off easy and build up the intensity slowly over time giving your body a chance to adapt. We apply these same rules to chiropractic adjustments. We never try to force anything…even if the patient wants us to…it's just not worth it.

So YES…chiropractic is safe. It's also very gentle and effective…and affordable too.

Chiropractic is also very efficient. A typical chiropractic patient will usually have multiple areas of concern…such as neck pain, low back pain, and shoulder pain. A chiropractor can work on all three areas at the same time. This is why chiropractic patients are often amazed when they first discover chiropractic. Amazed that so many seemingly unconnected chronic ailments can start to feel so much better so quick.

Update April 13, 2012: Safety of Adjusting. No one pays closer attention to injury statistics than Malpractice Insurance carriers. Scott Haldeman, M.D., D.C. reviewed malpractice claims records for a 10-year period between 1988 and 1997. In reviewing 134.5 million chiropractic adjustments, there were 23 cases of stroke or vertebral artery dissection. 10 of these had complicating factors such as high blood pressure, use of oral contraceptives, or a history of smoking, all associated with vascular disease. The incidence of stroke or vertebral artery dissection following neck adjustments was one per 5.85 million. On average, a chiropractor would have to work for 1430 years (48 full chiropractic careers) for this to happen once! Driving to the office is probably more dangerous. Canadian Medical Association Journal, 2001

Bottom Line: It's normal to wonder if chiropractic is safe…and it's smart to do your homework. Hopefully this information can help you make an informed decision. Do yourself and your family a favor…consider chiropractic…it's the smart thing to do!

Is Your Chiropractor a Health Resource?

Chiropractors in san francisco Health is a spontaneous occurrence when the conditions are right. As chiropractors it is our job to teach our patients what these conditions are…not just treat their back pain. Sure, chiropractors do a great job of treating back pain…In fact, Consumer Reports ranks chiropractic #1 for back pain treatment.

Personally, if all I did was treat back pain or just musculoskeletal problems, life as a chiropractor would be boring. I learned early in my career that the chiropractors that seemed to have the largest most successful practices were the ones that did more than treat back pain…they were trusted health resources.

After-all, according to original chiropractic doctrine, chiropractic, which means "done by hand", is first and foremost a philosophy. The Philosophy of Things Natural. Chiropractic is based on the premise that what's natural is perfect. You cannot improve upon nature. You can either work with it…or against it. Working with it steers us in the direction of health. Working against it steers us in the direction of illness.

The body does not need outside intervention to be healthy…it is it's natural state. What we don't need are things that interfere with with our natural state. Things like processed foods, high fat diets, drugs, alcohol, tobacco, pollution, sedentary lifestyles, stress, lack of rest, and Vertebral Subluxation.

Vertebral Subluxation is a condition that results from physical, chemical, or mental stresses that the body is unable to adapt to, resulting in sickness and dis-ease…and yes back and neck pain. It's what chiropractors treat…we coined the term.

The nervous system is the first organ to develop in utero. Then the nervous system controls the development of the rest of the body…and continues to control all bodily functions from birth to death.

The nervous system is so important that it is the only organ in the body to be completely encased by bone to protect it. The vertebrae is cut-up into 24 move-able segments called vertebrae. When the bones are in alignment (their natural state) they protect the nerves as they branch off the spinal cord and travel to the rest of the body. Every cell, tissue, gland and organ in the body has a nerve connected to it directly or indirectly (hormones and blood cells) controlling their function.

When you suffer a vertebral subluxation, which can actually occur during the birth process (how many of us were born?) it can interfere with your natural state of health and result in symptoms such as back pain, neck pain, digestive problems, irregular heart beat…it just depends where the nerves are  going that are being interfered with. Sometime there are no symptoms at all. Which is why it makes sense to get checked about once a month…with or without symptoms.

Chiropractors locate, identify, and correct subluxations…it's what we do.

We also teach our patients about how to prevent what we treat. What I call "creating the conditions of health". Here are the components of a healthy chiropractic lifestyle:

1. Proper nutrition with an emphasis on natural foods such as grains, fruits, vegetables.

2. Plenty of rest, preferably with a cervical pillow.

3. Aerobic and strength training (lots of core exercises).

4. Stress Management such as visualization/meditation.

5. Regular chiropractic adjustments and check-ups.

6. Lots of pure H20 (1/2 your body-weight in ounces per day).

7. No smoking, drugs, or excessive alcohol.

A chiropractor should be more than just a back pain doctor…they should be a health resource. They should have a rolodex of information they can share with their patients to help steer them in the direction of health. This blog is part of my rolodex. Honestly, I can't imagine what it must be like to be only a pain doctor. Pain is a symptom not the actual problem. And so are just about all health problems.

Adjust your lifestyle choices and create the conditions of health…and watch your symptoms dissapear…because heath is your natural state.

NOTE: I know there is a limit to how much we can recover from certain ailments, no matter how perfect an environment we create for healing. But that doesn't mean we shouldn't strive to reach our health potential. Your chiropractor can help.

Got Neck Pain? Consider Chiropractic

Neck pain treatment san francisco Neck pain is very common, effecting up to 70% of us at some point in our lives. While there are specific causes of neck pain such as a sports injury, whiplash, or sleeping in a funny position…most of the time no direct cause can be identified. The term for this is "non-specific neck pain" or "neck pain of unknown origin".

Doing the same repetitive motions day in and day out such as working on a computer and rotating your head back and forth from the computer monitor to a worksheet or between monitors like traders do, can lead to repetitive stress injuries (RSI) or cumulative trauma disorders (CTD).

The stress builds up silently over time in the neck joints, muscles, ligaments, and tendons, and eventually causes neck pain and disability.

The good news is, research shows that 100% natural chiropractic care is able to help the majority of neck pain patients and should probably be your primary treatment choice.

The ability to predict a favorable response to treatment has been termed, “clinical prediction rules” which in general, are usually made up of combinations of things the patient says and findings from exams. In a large study, data from about 20,000 patients receiving about 29,000 treatments, was collected and analyzed to find out what complaints responded well to chiropractic treatment.  The results showed that the presence of any 4 of these 7 presenting complaints predicted an immediate improvement in 70-95% of the patients: 1. Neck pain; 2. Shoulder, arm pain; 3. Reduced neck, shoulder, arm movement; 4. Stiffness; 5. Headache; 6. Upper, mid back pain, and 7. None or one presenting symptom.  Items not associated with a favorable immediate response included “numbness, tingling upper limbs,” and “fainting, dizziness and light-headedness in 4-12% of the patients.  The “take-home” message here is that was far more common to see a favorable response (70-95%) of the patients compared to an unfavorable response (4-12%), supporting the observation that most patients with neck complaints will respond favorably to chiropractic treatment.

How long it takes to fix your neck pain depends on several factors.  Your chiropractor will want to know things such as whether or not your neck pain is new and acute (sports injury/whiplash), reoccurring, chronic, or involves repetitive motions (RSI or CTD). A chiropractor specializing in neck pain will also want to know if you use a cervical pillow or not, the type of mattress you sleep on, your ergonomic set-up, and your hobbies…to name a few. Every case is different.

Some neck pain patients respond in a very short period of time, such as days…some take weeks or months to respond. An expert chiropractor is able to make recommendations based on your initial consultation, case history and exam findings. Often times a cervical pillow, ice, and exercises are prescribed to speed up the healing process and prevent future problems.

The Graston Technique® Instruments, much like a tuning fork, resonate in the clinician's hands allowing the clinician to isolate adhesions and restrictions, and treat them very precisely. Since the metal surface of the instruments does not compress as do the fat pads of the finger, deeper restrictions can be accessed and treated. When explaining the properties of the instruments, we often use the analogy of a stethoscope. Just as a stethoscope amplifies what the human ear can hear, so do the instruments increase significantly what the human hands can feel.
