Most of the patients we treat in the SF Financial District spend the majority of their day sitting in front of a computer doing repetitive motions. This activity can put a lot of stress on the bones of the spine in the neck and well as the muscles, ligaments and tendons of the shoulders, arms and hands.

The nerves that control the arms and hands exit from in between the bones of the spine in the neck. These nerves can become compromised with abnormal wear and tear leading to numbness and tingling in the hands. The problem can be in the neck, shoulders, elbows, or wrists.  All must be checked.

In most cases there is joint dysfunction and muscle imbalances, and even nerve pressure in multiple locations. The good news is that these sorts of problems tend to respond favorably to chiropractic and favorable outcomes are the norm.

Neck Pain Relief in San Francisco

Neck pain is very common in people who spend the majority of their day sitting in front of a computer doing repetitive motions, which is what most of the work force does in the Financial District.

There are 3 basic types of neck pain:

  1. New and acute neck pain.
  2. Chronic neck pain (over 90 days duration).
  3. Repetitive Stress Injury (builds-up silently over time).

While most of the new patients we  see for neck pain report that their neck pain is new, it turns out that the problem has been building up silently under the surface due to things like poor ergonomics, stress (tenses neck muscles), poor sleeping habits (lack of cervical pillow), inadequate breaks at work (it’s best to take frequent stretch breaks), poor posture.

Sure, you can be using a cervical pillow, taking micro-breaks at work, meditating, and have a perfect workstation set-up and still get neck pain, but the odds are greatly reduced when these things are in place.

So what’s the best treatment for neck pain?  

Well, according to research, chiropractic and low tech exercises (the kind chiropractors show you) is much more effective than OTC medications or physical therapy. It’s much more cost effective and takes less time as well. Here is an excerpt from Chiro-Trust.org entitled Chiropractic & Exercise vs. OTC Medication for Neck Pain:

However, this new research clearly supports that seeing a chiropractor and/or engaging in light exercise can bring neck pain relief more effectively than relying on pain medications! Researchers even found that the benefits of chiropractic adjustments were still favored A YEAR LATER when comparing the differences between the spinal manipulation and medication treated groups! Moderate acute neck pain is one of the most frequent complaints prompting appointments at primary care/medical clinics and is estimated to account for millions of doctor visits per year. In some cases, pain and stiffness occurs without a known cause and there is no “standard” medical treatment. Though physical therapy, pain medication, and chiropractic have all been utilized for neck pain, until now no one had compared the benefits of each in a single study.

The study consisted of 272 neck pain subjects split up into three groups: 1) Chiropractic group (approximately 20-minute treatments an average of 15 times); 2) Pain medication group (meds included acetaminophen, and in some cases stronger prescription meds including narcotics and muscle relaxants); 3) Physical Therapy group (consisting of meeting twice and receiving advice and exercise instruction at 5-10 repetitions up to eight times a day).

At the end of three months, the chiropractic and exercise group did significantly better than those who took drugs. Approximately 57% of those receiving chiropractic management and 48% of those who did the exercises reported at least 75% reduction in pain vs. 33% of people in the medication group. A year after the treatment period ended, the numbers decreased to 53% in the chiropractic and exercise groups, compared to 38% in pain medication group. The chiropractic group received the highest scores in patient satisfaction at all time points. An interesting downside noted in the medication study group was that the subjects had to use a progressively greater amount of medication at a progressively increased frequency to manage their pain. Stomach trouble is the most common side effect of NSAIDs (leading to ulcers) as well as liver and kidney problems. Another interesting finding was that the subjects in the medication treated group felt less empowered, less active, and less in control over their own condition compared with those in the other two groups.

Headaches From Sitting at the Computer?

Headaches Relief in San Francisco

We have been treating headaches in San Francisco for over 23 years.  

Headaches are fairly common…especially with people that spend a lot of time on the computer like most of our chiropractic patients in the San Francisco Financial District.

Sitting for long periods in front of a computer can cause static muscle tension in the back, neck and shoulder muscles. Constant tension on the neck and shoulder muscles which attach to the head can cause tension headaches that can become chronic (lasting over 90 days) and debilitating.

Poor ergonomics can also contribute to computer related neck pain and headaches. As can lack of proper (frequent) micro-breaks, which are brief resting periods (typically 1-2 minutes) that allow the muscles to rest and recover.

In addition, you could be compounding the problem by sleeping on your stomach or using a soft pillow. It’s always best to sleep on your back or side with an orthopedic pillow like a Tempur-Pedic Pillow.

If you do get a sore neck at home there are stretches you can do as well as ice techniques to reduce inflammation, pain and muscle spasms.  Your chiropractor is your best resource for these.  You can also search Google or YouTube. What is Carpal Tunnel Syndrome?

Here is an excerpt from an article on the great chiropractic consumer website Chiro-trust.org entitled Carpal Tunnel Syndrome-What Is It?:

Carpal Tunnel Syndrome (CTS) basically occurs when pressure is applied to the median nerve as it travels through the wrist on the palm side resulting in numbness, tingling, pain, and later, weakness of the grip and pinch functions. But, the median nerve can be pinched at many other locations as it courses down from the neck to the hand, which is why we examine and treat the CTS patient from the neck down! The median nerve has been described as the “eye of the hand,” as it is one of the three major nerves formed from the brachial plexus– that “highway” of nerves made up of the C5-T2 roots leaving the neck, merging together to eventually form the three main nerves of the arm. Because the median nerve function regulates pinch and grip strength, buttoning a shirt, writing a note, driving a car, and even sleeping are ALL affected by a median nerve pinch.

So, since the median nerve originates in the neck, and carpal tunnel symptoms are mostly in the hand, the neck-hand connection is often overlooked by doctors and therapist that do not routinely treat CTS.

Our carpal tunnel doctors in San Francisco have been treating CTS since before the dotcom boom-bust. Cervical Disc Treatment has evolved over the years to the point where neck surgery is not the primary option. Is it Carpal Tunnel Syndrome or the Neck?

We have been providing treatment for carpal tunnel syndrome in San Francisco for over 22 years.

One of the more common presentations of symptoms that we see at our chiropractic clinic in downtown San Francisco is computer related hand pain, numbness, tingling, burning, aching and weakness (some or all of these together). Sometimes there is associated neck shoulder, or arm pain.

So the question becomes…what is the underlying cause of the symptoms?

Sure, these are the classic symptoms of carpal tunnel syndrome (CTS), but the likelihood there is true CTS is rare. What’s more likely is that the cause of the CTS symptoms is a pinched nerve in the neck. After-all…the nerves that exit from in between the bones in the neck, travel into the upper extremities and innervate and control the arms and hands.

When there is pressure on these nerves in the neck it can cause the same signs and symptoms of true carpal tunnel syndrome.

Pinched nerves in the neck are mostly the result of bulging or herniated cervical discs, subluxated vertebrae, or degenerative disc disease…all of which respond favorably to chiropractic adjustments. In the event we need a little help, we have deep tissue laser or nonsurgical spinal decompression.

We do chiropractic adjustments to the neck, shoulders, elbows, wrists and fingers on both sides. We do massage and exercise as well. We are also concerned with proper ergonomics, sleeping positions and nutrition. It is this comprehensive approach that has allowed us to successfully treat thousands of patients with CTS symptoms over the years.

Hand Pain Treatment in San Francisco Downtown

We have been providing natural hand pain treatment in downtown San Francisco for 22 years now in the Financial District. 

Most of our patients spend the majority of their day sitting in front of a computer doing repetitive movements with their arms and hands.  In addition, they are mostly under the stress of meeting deadlines and are expected to perform at a very high level.

On top of this, many of our patients are actively involved in sports and hobbies that require repetitive use of the upper extremities like weight lifting, cross-fit, cycling, and tennis.

This is the reason hand pain is so common with office workers and urbanites. Not to mention the fact that most of us then go home and spend more time on the computer. In fact, many are connected on the way to and from work. OUCH!

In my experience, the source of the hand pain is typically a combination of pressure on the nerves in the neck that innervate (connect to) and control the hand, and pressure in the wrist on the median nerve in the carpal tunnel. This is called “Double Crush Syndrome” and is one of the more common conditions we treat.

Many doctors and physical therapist have a good understanding of this condition but lack the ability to successfully treat it. In my opinion…and I have treated provided hand pain treatment to over 5000 patients, treatment must include the neck, shoulders, elbows, wrists, and fingers on both sides. It is also important to make sure the rest of the spine is in proper alignment and the legs are even. A short leg can cause muscle tension in the neck and lead to pinched nerves.

Our hand pain treatment will usually include chiropractic adjustments to the neck, shoulders, arms, and hands, as well as myofascial release, cervical traction, custom exercises, and ergonomic instruction. We will also address sleeping postures and nutrition. It all matters.

A hand pain treatment visit at our chiropractic center takes about 30 minutes. In the event of severe hand pain or injury from trauma, we may refer you to one of our physical therapist or medical doctors , either onsite or at another location. We also may need to take x-rays or perform diagnostic tests to quantify your problem.

The bottom line is this…when it comes to hand pain treatment not all chiropractors, medical doctors or physical therapist are equal…experience matters!

Whiplash: What’s Best Rest or Treatment?

Whiplash Treatment in San Francisco

We have been providing treatment for whiplash in downtown San Francisco for over 20 years.

Whiplash, or WAD (whiplash associated disorders) results from the rapid movement of the neck and head resulting in injury. This is the net result of the “classic” motor vehicle collision, though other injury models (like slips and falls) can result in similar injuries. Last month, we listed basic facts, of which one was better results (less long-term pain and disability) occurred from initial active treatment of the neck with mobilization/manipulation, exercise, and encouraging movement vs. placing a collar on the patient and “resting” the injured neck. Though there are a few studies that suggest there is no difference in results, the majority state that it is BEST to actively treat the patient and encourage movement (of course, assuming no unstable fractures have occurred) rather than to place the patient into a collar and limit activities. The first question that we’ll address this month is, why is this important?           

            The simple answer is that you, as an advocate for an injured friend, family member or as a patient yourself, may NOT be offered “the best” treatment approach by the ER or primary care physician. In fact, one study cited a survey regarding the management of whiplash injuries in an ER and reported that between 23-47% of physicians prescribed a soft cervical collar for acute whiplash rather than promoting immediate active treatment. By knowing this information, the knowledgeable patient can refuse the collar method of care and seek care that emphasizes the use of early mobilization and manipulation, like chiropractic! Though referrals to chiropractors are increasing as more research becomes available, chiropractic care is still significantly ignored or not considered by many practicing ER and primary care physicians. As always, you need to be your own “best advocate,” and the only way to do that is to be informed, hence the intention of this Health Update! Some studies even report that the use of a collar may have deleterious or “bad” side effects and can actually make you WORSE (this was reported by the Quebec Task Force)! The majority of studies on the subject of whiplash report that encouraging “normal activity,” as opposed to immobilization, IS the best approach. We will certainly help steer you in the right direction!

            Next, let’s talk about WHY does this method works better? The research supports that soft tissues injuries heal better and with less scar tissue formation when patients receive active treatment/early activity types of care (like manipulation / chiropractic). In general, any treatment approach that reduces patient suffering sooner, encourages one to return to “normal activities” faster, and promotes independence and self-care methods earlier is the best approach!
