Pinched Nerve in the Neck?

One of the most common causes of neck, shoulder, arm, and hand pain is a “pinched nerve in the neck”. A pinched nerve is medical terminology (formerly doctor slang) for pressure on a nerve or nerve compression.

Here is how the Mayo Clinic defines a Pinched Nerve:

A pinched nerve occurs when too much pressure is applied to a nerve by
surrounding tissues — such as bones, cartilage, muscles or tendons. This
pressure disrupts the nerve’s function, causing pain, tingling,
numbness or weakness.


A pinched nerve can occur at several sites in your body. A herniated
disk in your lower spine, for example, may put pressure on a nerve root,
causing pain that radiates down the back of your leg (sciatica).
Likewise, a pinched nerve in your wrist can lead to pain and numbness in
your hand and fingers (carpal tunnel syndrome).

Me: A pinched nerve in the neck is usually the result of a herniated or bulging cervical disc, vertebral subluxation (misalignment), degenerative disc disease, facet syndrome, or moderate to severe sprain/strain from say a whiplash or sports injury (think football). This can happen all at once from a trauma or slowly over time from sitting in an awkward position at your workstation, poor sleeping habits (flat pillow with no support), or hobby’s like bowling, playing a musical instrument, knitting, or gardening.

These days, just about everyone spends a lot of time on the computer. Our chiropractic clinic is located in the SF Financial District so almost all our patients sit at a desk for very long hours performing repetitive tasks with the upper extremities on the computer. Many then go home and do more of the same. And a very high percentage of these people already had misalignments in the neck from years of school, studying, sports and life. The stress of working at the computer all day is just another layer of the onion that can accelerate the degnerative process and lead to pinched cervical nerves.

So, what can be done to prevent pinched nerves from happening and what is the best treatment once they do happen?

Well, I am a chiropractor, and I specialize in the treatment of neck, arm, and hand pain. So my opinion is biased owards chiropractic care and nonsurgical spinal decompression. Of course, most of the patients I treat are already experiencing pain and dysfunction. So, our primary focus is to treat the problem and get some relief. We do this using a combination of chiropractic adjustments to the neck, shoulder, arms and hands. We incorporate massage and stretching into the mix, as well as custom low tech exercises. If needed, we use deep tissue laser therapy to reduce inflammation and pain levels.

If a pinched nerve is due to a herniated or bulging disc and the patient does not respond to conventional chiropractic as described above than we will consider cervical spinal decompression with the DRX9000c. The DRX9000c is designed and built to treat cervical disc herniations naturally.

Sometimes we use a combination of everything we have to achieve favorable outcomes. Our 20years+ experience comes in handy when it comes to making decisions on pinched nerve treatment.

San Francisco Chiropractor Review: Back Pain

Anthony EspirituThe Executive Express Chiropractic "Patient of the Week" is Anthony Espiritu. Anthony presented to our clinic with chronic low back pain due to degenerative disc disease. Can Sitting Long Periods Cause a Disc Herniation?

Xxx Most of the patients we treat at Executive Express Chiropractic in downtown San Francisco, spend the majority of their day sitting in front of a computer. Sitting for long periods can be very hard on the back and neck…especially on the spinal discs.

There is a right way to sit and many wrong ways…and depending on the physical condition of the body sitting in the chair…a herniated disc may be waiting to happen.

Our bodies simply are not designed to be sitting in a chair all day…they are designed to be moving around…they need motion for all the joints and spinal discs to stay healthy. Spinal discs and joints are avascular…meaning there is no direct blood supply to them. They receive nutrients from the bloodstream and expel waste products through a pumping action caused by motion…this process is called imbibition in the spine.

Often times sitting for long periods in conjunction with bad posture and poor lifestyle choices, such as…lack of exercise, poor diet, bad posture, not enough rest, too much stress (tight muscles), being overweight, using a bad pillow, etc…can lead to abnormal stresses on the lumbar or cervical spinal discs, and over time contribute to spine problems such as bulging and herniated discs.

Heck…you can even be a picture of health…but if you are in front of that computer for 10-12 hours per day with minimum breaks, working under intense stress…you are at risk.

So what should you do if you sit all day to prevent spine problems like disc herniations from happening?

Well…you should have a good ergonomic chair and proper workstation setup, you should take micro-breaks throughout the day, you should get plenty of exercise…including specific spinal exercises (from a chiropractor), use a cervical pillow when you sleep…have a good mattress, a good diet, and try to manage mental stress. In addition, chiropractic adjustments work wonders keeping the spine healthy and better able to handle just about anything. The more all of the above is out of balance…the more spinal adjustments you should get.

Back Pain That Comes and Goes?

Images[1] If you have back pain that comes and goes…and it's always in the same spot…chances are it's because of pressure on the nervous system. The source of the pressure is most likely a vertebral subluxation, a bulging or herniated disc…or degenerative disc disease…or spinal stenosis.

How do you know this…couldn't it be tight muscles or something else?

Yeah…I guess it could…but in my (17 years as a San Francisco Chiropractor) experience that's usually not the case. Yes…there are muscle spasms associated with any of the neurovascular compression syndromes listed above…but the muscle spasms are not the primary cause.

Sure, you can pull a muscle in your back doing something…then it goes away…that's not what we're talking about here. We are talking about the same kind of back pain, in the same location, that keeps reoccurring.

Don't get me wrong…muscle spasms, scar tissue, and fibrosis must be addressed also…we do this with Active Release Techniques (ART). 

Typically…when a neurovascular compression syndrome is left uncorrected…it continues to get worse over time…so the frequency of episodes increases, as does the intensity of the pain…and the duration of each episode.

So…what should I do if this is happening to me?

Well…you should find yourself a good chiropractor. The best way to find a chiropractor is word of mouth or online research. I write this blog as an educational tool for my existing patients and staff…and to help give prospective chiropractic patients information that they can use to help understand the benefits of modern chiropractic care.

Degenerative Disc Disease: Is it a normal part of aging?

Images[2] Degenerative Disc Disease (DDD) of the spine is very common. In fact…some experts say it's a normal part of the aging process.

My opinion is this…disc degeneration occurs because of physical, chemical, and mental stresses (lifestyle choices) that the body is unable to adapt to…it's an adaptation…a response…just like any other degenerative condition.

When we put abnormal stresses on the spine from say years of poor posture, stress, lack of exercise, and a bad diet, or drugs and alcohol…it degenerates…and so does everything else in the body.

Sure, it's possible to be in perfect health, have a normal bodyweight, a great diet, exercise frequently, and have good posture…and still have disc degeneration. It's not only possible…but it's fairly common…WHY?

Well,  discs wear abnormally when the spine is out of alignment, just like a tire wears abnormally when the front end of you car is out of alignment. This is why it's SO important to make sure your spine is in it's proper position (hint: Chiropractic). This alone can cause DDD.

And even if you are a picture of health…you may have had an incident in your life (ever fall when you were a kid?) that caused a spinal bone to misalign, leading to abnormal wear and tear on a spinal disc…setting the stage for a serious spinal problem down the road…and you didn't even know it.

Poor lifestyle choices and an unhealthy internal context would just make the problem worse…kind of like throwing gasoline on a fire. Everything matters…what you eat…what you think…how you setup your workstation…the pillow you use…your exercise routine…your spinal hygiene…etc.

Sure, degenerative disc disease is common…but so are poor lifestyle choices. There is a direct correlation (in my opinion).

BOTTOM LINE: A healthy lifestyle and proper spinal alignment=A healthy spine.

Degenerative Disc Disease Has A Cause

Images[1] Degenerative Disc Disease (DDD)has a cause…just like any other degenerative disease. However, there are still a lot of doctors out there that consider disc degeneration a normal part of the aging process. Sure, there are lots of people with DDD, and it is common…but we don't need to re-define what should be considered normal…because there are ways to slow it down.

I have been practicing chiropractic in downtown San Francisco for almost 17 years. During this time I have seen thousand and thousands of spinal x-rays and MRI films. Yes, DDD is common…but it's a lot less common…and less severe in patients with healthy lifestyles and proper spinal alignment.

Yes spinal alignment (and posture) matter. When the spine is in alignment, the spinal discs wear in a more normal fashion….just like the tires wear normally when the front end of your car is in alignment.

The spine is composed of 24 movable segments called vertebrae. When a vertebrae moves out of position we call it a Subluxation…which is what chiropractors treat. Regular chiropractic care can help preserve the spinal discs and reduce the odds of disc disease and spinal degeneration.

The other cause if degenerative disc disease (DDD), just like any other degenerative disease is poor lifestyle habits…things like poor diet, smoking and alcohol, lack of exercise, mental stress, not enough sleep, bad mattresses, and on and on.

The spine and spinal discs are made up of living tissue just like the rest of the body…these cells reflect the quality of their context. If the context is not healthy, neither are the cells.

Sciatica Treatment With Spinal Decompression

Md_news_photos_011 Sciatica, or pain down the leg, is usually the result of herniated discs in the lumbar spine. The herniated or bulging disc puts pressure on the lumbar nerve roots that exit from in between the vertebrae and cause leg pain (Spine-health sciatica video).

There are other cause of sciatica, such as vertebral subluxation, degenerative disc disease, spinal stenosis, and piriformis syndrome…even facet syndrome. But a herniated lumbar disc is the primary cause…at least that’s been my experience over the past 17 years.

Anyway, other than cortisone and surgery, there just isn’t much in the form of traditional medical treatments. Both of these have limited success, side effects…and risks…that many patients don’t want to take.

Traditional chiropractic can be quite effective for herniated discs…but sometimes it wont help at all…or very little. So what can you do?

Osteoarthritis Exercises

184871 Osteoarthritis is a form of arthritis (inflammation of a joint) that is caused by a breakdown of cartilage in the joints…including the joints of the spine.  Osteoarthritis is also know as degenerative joint disease, or DJD.

DJD usually occurs in weight bearing joints like the knees, hips, and spine…but can occur in other joints like the wrist and fingers.

The cartilage loses it’s elasticity with DJD, which can cause the muscles, ligaments, and tendons to become stretched and overworked…a chain reaction. This can lead to chronic pain, inflammation, and loss of motion.

Chiropractic care can often times help DJD…but one of the best things you can do is exercise. Some exercises can make you worse, so you have to be careful. A good spine doctor or physical therapist will usually know the right exercises to prescribe.
