Managing Low Back Pain

We have been providing treatment for low back pain in San Francisco for over 22 years.

Many will develop chronic low back pain which is back pain that persists for over 90 days. Some will develop low back pain that goes away…but then it returns and becomes worse. Some get low back pain that goes away and never returns…and everything in between. Just about every case is unique and should be treated as such.

Sciatica, which is pain down the leg or legs due to pressure on the sciatic nerve can also result from the same underlying causes of low back pain. In fact, you can have sciatica and not have back pain at all…just the pain, numbness, or abnormal sensations in the leg. Most patients with low back pain just have low back pain with varying levels of severity.

The causes of low back pain are many. Things like obesity or weight gain can put too much stress on the low back and cause low back pain. If this persists it can result in thinning of the discs, disc degeneration, bulging and herniated spinal discs, and spinal stenosis. Since 2/3 of Americans are overweight or obese this is a primary cause.

Then there are injuries to the spine from sports, exercising, car accidents, poor posture and ergonomics, bad sleeping habits, slips and falls, traveling, and other activities of life.

Here are some of the more common conditions that cause Low Back Pain:  

  • Lumbar sprain or strain
  • Herniated or Bulging Disc
  • Spinal Stenosis
  • Facet Syndrome
  • Scoliosis
  • Degenerative Disc Disease
  • Vertebral Subluxation and Misalignment
  • Spondylolisthesis

The good news is the low back pain responds very favorable to chiropractic adjustments. This is good because often times the true cause is not known. That’s right…you can take 10 different doctors, physical therapists, chiropractors, medical, whatever, and get 10 different opinions. I sure would not want to be getting back surgery under these conditions. But chiropractic…sure, why not? Chiropractic is 100% natural and very very safe…and gentle too.

Heck…all you care about is getting rid of your low back pain right? Not many patients care about the name the doctor gives it…and why should you?

Here is an excerpt from an article entitled The Mysteries of Low Back Pain on the great website Chiro-Trust.org

Do you realize how complicated the low back region is when it comes to investigating the cause of low back pain (LBP)? There can be findings on an x-ray, MRI, or CT scan such as degenerative disk disease, arthritis, even bulging and/or herniated disks that have NOTHING to do with why the back hurts. Similarly, there are often other abnormal findings present in many of us who have NO low back pain whatsoever! Because of this seemingly paradoxical situation, we as clinicians must be careful not to over-diagnose based on the presence of these “abnormal findings” AND on the same hand, be careful not to under-diagnose them as well.

Looking further into this interesting paradox, one study reported findings that support this point. Investigators examined 67 asymptomatic individuals who had NO prior history of low back pain and evaluated them using magnetic resonant imaging (MRI). They found 21 of the 67 (31%) had an identifiable disk and/or spinal canal abnormality (which is where the spinal cord and nerves run). Seven years later, this same group of non-suffering individuals were once again contacted to see if they had developed any back problems within that time frame. The goal of the study was to determine if one could “predict” who might develop low back pain based on certain abnormal imaging findings in non-suffering subjects. A questionnaire was sent to each of these individuals, of which 50 completed and returned the questionnaire. A repeat MRI scan was performed on 31 of these subjects, and two neurologists and one orthopedic spine surgeon interpreted the MRI studies using a blinded approach (without having knowledge about the subject’s symptoms or lack thereof). Each level was assessed for abnormalities including disk bulging/herniation and degeneration. Those who had initial abnormal findings were defined as “progressed” (worsened) if an increased severity of the original finding was evident or if additional or new spinal levels had become involved over the seven-year time span.

Of the 50 who returned the questionnaire, 29 (58%) had NO low back pain. Full story here

Bottom Line: It’s easy to get all caught-up in a bunch of unnecessary diagnostic testing and doctors visits. My recommendation is to make the first visit to the chiropractor…it will most likely also be your last.

(FM) has quite a history! There is evidence that dates as far back as
Hippocrates in ancient Greece,
when he described a mysterious condition affecting the muscles which portrays
FM quite accurately!

            FM is a complex condition that
includes fatigue and chronic muscle, tendon, and ligament pain that is
wide-spread (NOT contained to one small region). Generalized pain and fatigue
are the hallmarks of FM, which can range from mild to severe disability.
However, there are many misconceptions about FM.

            How much do you THINK you know about
FM? Try the self-test below. For each statement, decide if the statement is
true or false. The answers can be found below…

  1. FM
    shortens one’s life span.
  2. A
    patient may see several doctors before finding someone willing to help them.
  3. FM
    can affect children, though it is most common in adults ages 20-55.
  4. To
    properly diagnose FM, you must have at least 11 of 18 tender points.
  5. With
    FM, you should not exercise if your body hurts.
  6. A
    multi-discipline treatment approach usually works best (chiropractic, primary
    care, massage therapy, and others).

            FM is present in 2-4% of the
population and affects everyone differently, so each case is best managed by an
individualized form of treatment. In other words, one treatment approach for
every FM patient is NOT the proper approach. For some, fatigue is the primary
issue, while for others, it’s the sleep disturbance, irritable bowel syndrome.
or bladder problems that require the most attention. There is frequently a
co-existing psychological condition that may include depression, anxiety,
and/or a stress-related condition, such as post-traumatic stress disorder. In
reference to the 10 statements above, FM does NOT shorten one’s life span, but
it does affect quality of life. The primary goal of treatment is to help the FM
patient gain control of their condition. It truly can take multiple visits to
different doctors before finding one that’s willing to work with you and
coordinate care with other “team” providers to give you the best quality care.
FM can affect children, though it’s rare – it’s primarily the 20-55 year old
age group with a 9:1 ratio favoring the female population. The “old” 11 of 18
tender point diagnostic requirement has been replaced by “widespread,
generalized pain,” NOT limited to specific points. Exercise is one of the BEST
self-management strategies and should be encouraged. Though a genetic component
has been identified, FM is NOT something you are “born with.” The pain
associated with FM is NOT indicative of muscle deterioration. FM is often
confused with other diseases and the diagnosis is made by eliminating the other
more easily diagnosable disorders. Chiropractic is a VERY important part of the
management team in the care of the FM patient.

Chronic Neck Pain Treatment and Chiropractic

Chronic neck pain treatment san franciscoWe have been providing chiropractic treatment for chronic neck pain in San Francisco for over 20 years. 

Over 70% of Americans will experience neck pain at some point in their lives. Many will develop chronic neck pain, which is neck pain that persists for over 90 days. The primary risk factor for developing chronic neck pain, is previous neck pain. This is why it's so important to get the right kind of treatment at the onset of neck pain.

 

Chronic neck pain can have many causes. Here are a few:

  • A Bulging or Herniated Disc in the Neck: This can cause pressure on the nerve leading to what we call a "Pinched Nerve". Some patients experience pain down the arm and into the hand.
  • Spinal Stenosis: This is a narrowing of the spinal canal from spinal degeneration. Often times the patient will notice that certain positions or activities cause neck, shoulder, or arm pain.
  • Degenerative Disc Disease: Often times the neck pain will come on gradually, and get worse over time. Certain events may trigger pain, such as activities that are out of a patients normal baseline. Even stress can trigger pain.
  • Osteoarthritis in the Neck Joints: Arthritis in the neck joints can cause pain that is worse in the morning and better in the afternoon or as the day progresses. Exercises often help.
  • Vertebral Subluxation: This is what chiropractors call a vertebrae that has lost it's normal relationship with the one above and the one below and is not functioning properly. This can lead to pain and dysfunction and all of the above.

There are other causes of neck pain…and there are many triggers, such as poor posture at work, bad sleeping habits, sports injuries, too much stress, too much computer use, iPad, cell phone, etc. Traveling is also hard on the neck. Heck, working in your yard, playing with your kids, painting, working on your car, in your garden, or doing anything you do don't normally do…can all set the stage for neck pain, that can eventually become chronic.

So what's the solution…What is the best treatment for chronic neck pain?

According to recent research (see news story about chiropractic for neck pain), the best treatment for neck pain is chiropractic and low tech exercises. This is exactly what we do at our San Francisco Chiropractic Clinic…and frankly, it's what the best chiropractors do. 

We prefer to do the exercises at the clinic at the time of the chiropractic adjustment, and at the same time provide the patient with neck exercises and stretches they can do at home and at work. 

We also incorporate soft tissue methods such Active Release Techniques (ART) and Graston Technique to break up scar tissue, muscle spasms, and adhesion's. This in conjunction with chiropractic adjustments and low tech exercises is very powerful.

We are also concerned with educating our patients on proper ergonomics, sleeping position and use of a cervical pillow. It is also important to learn about things like micro-breaks, which are brief resting periods when working…this is a good time to stretch. Proper hydration is important too…this keeps the joints lubricated. Supplementation can help as well. Pharmaceutical Grade Fish Oil can help reduce inflammation in the joints, as can a diet loaded with fruits and vegetables. Everything matters.

Why Executive Express Chiropractic?

We have been providing chiropractic in the San Francisco Financial District for over 20 years now. A little over 3 years ago we opened Executive Express Chiropractic at the Embarcadero Center.

We told the architect to build us a space using eco-friendly materials because we did not want to expose our doctors, staff, and patients to the many toxins that are inherent in most carpets, paints, and building materials. How Many Adjustments Do I Need For Neck Pain?

Neck pain doctors downtown sfWe have been providing neck pain treatment in San Francisco for over 20 years now. During this time we have learned much. The most important thing we have discovered is that everybody is different and needs to be treated as such.

For instance…you may have two 35 year old men present with neck pain from sitting at a desk too long…one of them gets better after a few chiropractic adjustments, the other takes 10. That's just the way it is.

Sure, we make treatment recommendations based on a patients age, exam findings, and cases we have treated that are similar, but still, as we get into the treatment each case responds differently.

Chronic neck pain…which is neck pain that has persisted for 90 or more days complicates things further. Now we are talking about pain that is deep rooted. The nervous system has been re-wired because of the chronic neck pain and other parts of the spine and body are compensating.

Typically, chronic neck pain can take several weeks to many months to fully resolve. Chronic neck pain is usually the result of repetitive stress or injuries that have not healed properly, such as a sports injury or whiplash. Headaches can develope as well as shoulder and arm pain…even low back pain as the body adapts to the neck pain and abnormal motion and function.

So…how many adjustments you need for your neck pain will vary on all the above.

The good news is that chiropractors deal with all these scenarios on a daily basis…it's what we do. And after 20 years we have seen a lot…enough to know what to do and what not do do. When it comes to neck pain it's best to get chiropractic adjustments right when you feel the pain. The last thing you want is for your neck pain to become chronic.

My recommendation…see a chiropractor ASAP after any kind of injury to the neck, such as a whiplash from an auto accident…or a sports injury…or if you sleep funny or work too hard. And get checked for misalignment's to your neck periodically…say once per month.

If you take care of your spine it will take care of you…and nobody needs a pain in the neck!
