More Whiplash Facts and Advice

Last month, we discussed car safety
features, proper headrest position, how to prepare for a crash, and to seek
immediate treatment as ways to minimize the chances of suffering from whiplash
in the event of an accident. Here are four more ways to avoid or minimize
whiplash:

PAY ATTENTION WHILE YOU DRIVE. Too often, we
get distracted while driving. Any time our eyes leave the road, the potential
for an accident increases significantly. This can occur when changing the radio
to a different station, eating while driving, reading while driving, talking on
the phone, texting (equals the effects of 2-3 alcoholic beverages), driving
under the influence of certain prescription medications (pain killers for
example), driving under the influence of alcohol or other chemical agents, and
turning your head during conversation. We have a responsibility when we are
driving to keep our eyes on the road, as many accidents occur within split
seconds of time. If we are not paying attention, we will not be able to avoid a
potential collision.

THE CONCEPT OF "NO CRUSH, NO CRASH INJURY" IS SIMPLY
NOT TRUE
. In fact, just the opposite is true. That is, the
greater the amount of crushing metal at the time of collision, the greater the
amount of energy absorption that occurs, resulting in less force transferred to
the contents inside the vehicle (namely you or me). This is why, many times,
people are injured in low speed collisions because there is no energy absorbed
by crushing metal as noted by the absence of or, minimal damage to the car.

FOLLOWING YOUR DOCTOR'S ORDERS. It is very
important that we do not inadvertently hurt or harm ourselves further by NOT
following the advice of our health care practitioner. This means initially
using ice to reduce inflammation and swelling, possibly wearing a soft cervical
collar during the first few days after the injury to “rest” the injured
structures, following proper nutritional advice for optimal healing benefits,
and following exercise recommendations. This last treatment approach is vital
in the prevention of long term, chronic neck complaints. Other ways you can
REALLY help are to follow cervical traction orders. Cervical traction is a very
effective method of reducing muscle spasm, separating the joint spaces,
improving disk nutrient transfer and water content, reduce the pinching effect
of the nerves, and as a result, speeding up the recovery process. Follow your
treatment schedule; that is, DON’T SKIP APPOINTMENTS! During office visits, it
is necessary to discuss not only what is working well, but also what may not be
working so modifications to your care plan can occur on a timely basis. The
primary goal of whiplash management is to prevent the condition from becoming
chronic and long-term, and the first few weeks of treatment are critical!

ERGONOMIC MODIFICATIONS. An important part of managing
whiplash injuries is preventing daily irritations from tasks that we have to
do. Hence, we will discuss adjusting your work station for optimum positioning
and avoidance of poor posture, such as using office chairs with arm rests,
sitting posture modifications, and computer monitor positioning. Proper sleep
positions and pillow design are also reviewed.

Here is what the Mayo Clinic has to say about Whiplash:

Whiplash is a neck injury that can occur during rear-end automobile collisions, when your head suddenly moves backward and then forward — similar to the motion of someone cracking a whip. These extreme motions push your neck muscles and ligaments beyond their normal range of motion.

Whiplash injuries can be mild or severe. Treatment typically begins with over-the-counter pain relievers and ice applied to the painful neck muscles. If pain persists, prescription medications and physical therapy may be helpful.

Most people recover from whiplash in just a few weeks, but some people with whiplash injuries develop chronic conditions that can be extremely painful and disabling.

My Take: Sure, many people that suffer a whiplash injury to the neck or low back feel better after a short period of time…but have they really recovered from the injury? 

I have learned from personal experience (I had 3 whiplash injuries while in chiropractic college) and from 20 years of treating neck and back pain from whiplash trauma…that it's best not to leave musculoskeletal symptoms related to a car accident or other whiplash cause untreated.

What happens is the bones of the spine get knocked out of place and stay there…even though you feel better. In addition…the muscles, ligaments, and tendons that were injured from the whiplash, do not heal properly. They end up being weaker, less flexible, more pain sensitive, and prone to re-injury.

In order for the bones of the spine (and spinal discs) and the surrounding soft tissues to heal properly the right conditions need to be in place such as proper position and motion. Allowing the injured parts to heal on their own sets the stage for chronic neck or back pain down the road.

More often than not, patients that present to our San Francisco chiropractic center with chronic neck pain and headaches, have a history of whiplash injury from an auto accident.

It's sure a lot easier to treat a whiplash right after it happens than to treat a chronic back, neck pain, or shoulder pain syndrome years later.

Whiplash and Chronic Neck Pain

Whiplash treatment in san francisco We have been treating whiplash injuries in San Francisco for almost 20 years now. During this time, we have also had thousands of patients that have suffered a previous whiplash trauma from an auto accident (months or years prior). In fact, it's almost a daily occurrence.

The patients we see with a previous whiplash trauma tend to suffer with chronic neck pain, headaches, shoulder pain, and low back pain. Chronic neck pain is the most common residual effect of a previous whiplash injury…at least in my experience.

This is why it is so important to be checked by a chiropractor right after an automobile accident…even if you (or especially your children) do not feel any immediate symptoms.

Even a minor impact of say 5-7 mph can do serious damage to the spinal structures such as the muscles, ligaments, and tendons. The spinal bones and discs can also suffer injury from a low impact collision.

We hear something like this all the time "I was a little sore but then the pain went away, so I thought I was OK." Then years later, out of the blue, headaches develop or persistent neck pain. Then we end up finding muscle and ligament damage that we trace back to the car accident. Sure, we can help the neck problem now, years later…but it would have been a lot easier right after the accident.

I know from first hand experience that a whiplash injury is nothing to mess around with. I suffered three of them over a two year period. Thankfully, I went straight to the chiropractor each time.

So anyway…there IS a direct connection between whiplash injury and chronic neck pain.

We have been treating whiplash injuries at Executive Express Chiropractic (formerly Front Street Chiropractic) for 18 years now. 

 

The term ‘whiplash’ represents a collection of symptoms that occur as a result of a soft tissue injury of the neck.  This includes over stretching and/or tearing of muscles, tendons, ligaments, disk tissue and/or nerve injuries due to the extreme movements that occur during a whiplash event (usually arising from a car accident).  We have discussed the mechanism of injury and the symptom complex that can arise in past articles. 

So the question is – how many patients who sustain a whiplash injury actually improve and recover compared to those that don’t? In one study, it was stated that 43% of patients will suffer long-term symptoms after a whiplash type of injury. More specifically, if a patient is still symptomatic after 3 months following the injury, “…then there is almost a 90% chance that they will remain so.”  They go on to state that no conventional treatment has proven to be effective in helping these chronic cases.  The purpose of their study was to determine the effectiveness of chiropractic treatment in a group of chronic whiplash patients.  To do this, they studied 28 patients (20 women and 8 men, between ages 19-66, mean 39) over a 2-year time frame, injured in motor vehicle collisions. Their symptom severity was graded on an A to D scale (A=minimal symptoms vs. D=disabling symptoms, with B= nuisance and C=Intrusive or partially disabling). Those in Groups C & D either had to significantly modify their work or, they lost their jobs and relied on continual use of medications.  The chiropractic treatment included spinal manipulation (adjustments), controlled resistance of muscles to improve stability and coordination, and the use of ice.  Treatment from an emergency facility and/or their general practitioner and physical therapy had been previously utilized for on average 15.5 months, before entering this chiropractic-based study.  Initially, 27 of the 28 were classified into symptom groups C or D and symptoms included neck pain (82%), neck stiffness (36%), and other complaints of headache, shoulder, arm and back pain. Following treatment 26 of the 28 (93%) improved, 16 by one symptom group and 10 by two symptom groups and this degree of improvement was assessed and agreed upon by both an orthopedic surgeon as well as by a chiropractor.  Seventeen (61%) improved to a point of satisfaction where care was discontinued after the 1st assessment with 4 of the 17 considering return for treatment due to a return of symptoms.  Litigation was still pending in 20 of the 28 cases at the time the study concluded. 

This study is very important as over 90% of chronic whiplash cases improved from chiropractic management well beyond the point of improvement obtained through standard emergency, family practice and physical therapy.  Other studies have pointed out that early intervention or treatment with chiropractic manipulation and management approaches generally results in a more favorable response compared to waiting for longer time periods. To be able to obtain this level of success after an average of 15.5 months is truly remarkable!

Chiropractic methods often utilized for patients with a “whiplash” injury include spinal manipulation (or adjustments), mobilization techniques (this includes stretching, figure 8 movements, manual traction), muscle release work (this includes trigger point therapy, myofascial release/friction massage, and others), and promoting self-help approaches (this includes exercise, home traction methods, computer station modifications and other job modifications as indicated, and others).

If you have been in a car accident recently and are suffering from a whiplash injury…time is of the essence. You want to make sure you get the right kind of treatment for your neck pain (or low back pain) so that the injuries from the auto accident do not become chronic.

We will check for signs of soft tissue damage by feeling for areas of inflammation and pain. We will check your cervical and lumbar nerves to make sure they are functioning properly along with your muscle strength and relexes.

Your chiropractor will take all of this information along with the history of your accident and come up with a comprehensive treatment plan that will allow your body to heal with minimal loss of function.

How long this takes depends on the severity of the whiplash injury and any pre-existing conditions you may have already had.

In any case…the last thing you want to do is nothing…as this can lead to chronic neck pain or chronic back pain down the road. Chiropractic can still help with chronic pain…but it's much easier to deal with in the beginning right after the accident.  

So…if you have been involved in a recent auto accident or know someone that has…consider chiropractic care right away. And if have suffered injuries from a previous whiplash injury that were left untreated…it may be a good idea to get checked, as there may be problems that could pop-up down the road.

Whiplash, Cervical Disc Herniations, and Spinal Decompression

Md_news_photos_010 Most of the time cervical disc herniations develop gradually over time. But when you are in an auto accident and suffer a whiplash injury, it can happen all at once. In fact, even minor car accidents can cause disc herniations.

I have been helping cervical whiplash patients for 16+ years in downtown San Francisco. Most of the time chiropractic care helps patients return to almost normal.

But sometimes things don’t get better…or maybe only a little bit better.

Some of the symptoms of cervical whiplash are neck pain, shoulder pain, arm and hand pain…and headaches. Whiplash can also cause numbness and tingling in the extremities and weakness of grip.

If not treated properly, whiplash can lead to conditions such as degenerative disc disease and stenosis.
